It's back to school time and that means time for school sports physicals. In the past, the 341st Medical Group has conducted mass sports physical screenings, but in order to provide Malmstrom's student population with more personalized care, the Pediatric and Family Health Clinics are offering sports physical appointments daily.

For children enrolled in the Pediatric Clinic, sports physicals are accomplished during the annual well child exam.  If a child has had a well check since their last birthday, their sports physical form can be dropped off at the Pediatric Clinic front desk and the completed form will be available the following day. 

If a child hasn't had a well check since their last birthday, a well check appointment can be made with the Pediatric Clinic. Sports physical forms should be brought to the appointment. 

Since each school or camp has different physical requirements, it is the parents' responsibility to provide correct forms.  Parents should also complete as much of the form as possible prior to the appointment. Malmstrom Youth Program forms are available on-site.

A current shot record from the Malmstrom Immunization Clinic is required for all sports physical appointments. The 341st MDG Immunization Clinic is open Monday through Friday from 7:30 a.m. to 4:30 p.m.

The key to ensuring access to our pediatric population is to schedule sports physicals/well exams early. Sports physicals and well child exams can be scheduled up to 120 days in advance. It is advised to call as soon as possible to schedule these appointments.

Please remember only urgent care/emergency room and specialty care visits downtown are covered by Tricare.  If a child has received a school physical at a downtown facility, parents may incur a bill from Tricare.

Malmstrom Pediatric and Family Health Clinics use MiCare (Relay Health) secure messaging system. Individuals currently enrolled in MiCare can request Malmstrom Youth Program forms from the Pediatric Clinic and pick up completed forms the next day.  Sports physical appointments can also be requested through MiCare at https://app.relayhealth.com/.

Individuals not enrolled in MiCare can sign up in any of the patient care clinics.  MiCare is used to remind parents of upcoming wellness checks, send developmental screening forms and relay lab results prior to scheduled appointments.  MiCare can help save time at check in and allows parents to communicate with their child's healthcare provider without the need for "phone tag."
